(R3) Risk, Reliability and Resilience Post-COVID

The coronavirus, COVID-19 crisis has had a major impact on all industries, where the world has suffered immense damage from COVID-19 as a result of its scope and scale. In this course it is argued that systems of controlling similar types of disasters need to be improved through learning from past experience (in terms of failures and near missies) and from others (in the form of case studies and benchmarking), as well as through improved modelling for better decision making. Hence risk and reliability analysis coupled with resilience modelling are going to be discussed during this course.

Learning from major disasters provides techniques to explore the root causes of specific disasters and how we can learn from them. It focuses on a number of well-known case studies, in different industries including oil and gas, process industry, nuclear power generation, maritime industry and aviation. It is expected that be exposure to such case studies and the tools used to analyse them will lead to acquiring an ability to apply the same tools in your own industry.

Resilience can be characterised by abilities to absorb, adapt, and restore. It can be applied to both organisations and individuals, and during this course we will explore both applications. We will also investigate resilience modelling and the concept of the resilience triangle and the role of the 4R’s of robustness, redundancy, resourcefulness, and rapidity. These concepts will be demonstrated in the context of the covid-19 pandemic and how different countries have coped. This will be extended by including additional principles of resilience such as the principle of moving upstream.

Such modelling approach will be extended into the bowtie method, which is part of reliability analysis for capturing causal relationships. This will involve means of preventing hazards through safety barriers, or controls, as well as the consequences of the high-risk events and means of mitigating against them. The COVID-19 pandemic crisis will be modelled using a hybrid approach of bowtie and resilience triangle modelling as way of demonstration for crisis management.

Safety barriers are important strategic decisions for any organisation. We will focus on trying to answer the following crucial questions for different case studies: (1) were there barriers that were in place and how did they perform? (2) Were there barriers in place but not used? (3) Were there barriers that were not in place but were required? In managing pandemics, or any crisis situation, such barrier analysis helps in different ways such as encouraging explicit investments in safety barriers, raising awareness about characterisation and validity of safety barriers, assessing areas of vulnerability in the system and the need to incorporate new, or enhance existing, safety barriers, and it helps us to think about diversifying the nature of safety barriers, such as human, physical, software, and so on, so that risk is minimised against having one type of failure mode that can affect similar barriers simultaneously.
